Location: San Jose- Southeast
Schedule: Monday - Friday with on call weekends rotation
As a Home Health LVN, you will:
  • Provide skilled nursing care in accordance with the RN-established Plan of Care (POC), physician orders, and applicable LVN scope of practice.

  • Monitor and assess patient condition, identify changes or concerns, and promptly communicate updates to the patient, family/caregiver, physician, and supervising nurse.

  • Deliver safe, high-quality home health services while assisting patients with activities of daily living and promoting independence and self-sufficiency.

  • Educate patients, families, and caregivers on medications, treatments, health management, and self-care.

  • Maintain accurate, timely, and thorough clinical documentation in accordance with Lorian policies, procedures, and regulatory requirements.

  • Coordinate patient care with the interdisciplinary team, identifying and communicating the need for additional services such as PT, OT, ST, MSW, CHHA, RN, or dietary support.

  • Participate in case conferences and ongoing care coordination while using supplies and equipment efficiently and maintaining a patient-centered approach to care.

Required Qualifications
  • Certified/licensed to practice in the state of practice and in good standing.

  • Graduate of an approved school of practical/vocational nursing.

  • Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.

  • Possesses and maintains current CPR certification (BLS required).

  • Possess and maintains a valid driver's license.

This position is based on a pay per visit compensation model. Pay per visit ranges from $35 to $55 visit commensurate with experience and depending on type of visit.
