Job Title:Per Diem Case Manager / Registered Nurse (RN)
Location: New Jersey

Reports To: Director of Nursing
Job Summary:

Familyway Home Care is seeking a compassionate and experienced Perdiem Case Manager / Registered Nurse (RN) to join our team. This role is responsible for coordinating patient care, ensuring compliance with state and federal regulations, and providing oversight to home health aids. The Case Manager/RN will serve as the primary point of contact for clients and families, developing and implementing care plans to meet each client's unique needs.
Key Responsibilities:

Patient Care Coordination:

  • Assess, develop, and oversee individualized care plans for clients receiving home care services.
  • Conduct initial and ongoing assessments of patients to evaluate their health status and home care needs.
  • Monitor and ensure the quality of care provided by home health aids.
  • Provide education to patients and families on health conditions, medications, and self-care techniques.
Compliance & Documentation:

  • Maintain accurate and timely clinical documentation in accordance with state, federal, and agency guidelines.
  • Ensure that home health services comply with Medicaid, Medicare, and private insurance and Joint Commission regulations.
  • Conduct regular home visits to assess patient progress and ensure adherence to care plans.
Supervision & Staff Coordination:

  • Train, supervise, and evaluate home health aids to ensure they follow care plans and provide high-quality care.

Quality Improvement & Risk Management:

Qualifications & Requirements:

  • Active RN license in the state of New Jersey.
