As a Baylor Hospice LPN, you will carry out our Mission of Love by Assisting the RN Case Manager in carrying out the plan of care by providing skilled nursing care services.

Our Baylor Hospice LPN is responsible for the following:

  • Provide services in accordance with organizational policies and procedures and regulations that define scope of practice
  • Complete patient visits as assigned. Prepare the clinical and progress notes documenting outcomes of interventions
  • Assist the RN and/or physician in performing specialized duties.
  • Carry out the plan of care, including medication administration, pain assessments, dressings, treatments, and hands on personal care. Ensure supervisory visits are being completes by RN.
  • Prepare equipment and materials, and performs treatment adhering to aseptic technique as required
  • Attend IDG Meeting as needed in regards to patient visit collaboration.
  • Provide teaching to assist the patient/ caregiver in learning appropriate care techniques.
  • Assess patient/ caregiver response to care
  • Ensure communication of information to appropriate team members
  • Provide other job duties as assigned
  • Assist with Hospice consults as appropriate.
  • Assist with On-Call visits as needed.

What You Need as a Baylor Hospice LPN:

  • Unencumbered State license as a Licensed Practical Nurse
  • One year of nursing experience, preferably in Hospice, Home Health or a Skilled Nursing Facility
  • Current CPR certification (Basic Life Support, In-classroom training)
  • Must possess good verbal and written communication skills
  • Must be able to meet deadlines, work under limited supervision.
  • Must be able to perform cardiopulmonary resuscitation and relief of an obstructed airway; climb at least 2 flights of stairs and be able to stand and/or walk for a period of at least 4 hours
  • Valid PA driver's license and proof of motor vehicle insurance.
