• Deliver skilled nursing care to patients in their homes while promoting safety, independence, and quality of life.

• Complete comprehensive nursing assessments and develop individualized plans of care.

• Perform admission, routine, and follow-up visits based on physician orders and patient needs.

• Monitor patient progress and identify changes in condition, communicating findings promptly with physicians and the interdisciplinary care team.

• Educate patients and caregivers on disease management, medications, treatments, and self-care.

• Coordinate care with therapists, aides, physicians, and other healthcare professionals to ensure seamless patient care.

• Accurately complete clinical documentation in a timely manner.

• Maintain compliance with agency policies, HIPAA regulations, Medicare Conditions of Participation, and professional nursing standards.

• Deliver compassionate, patient-centered care that aligns with Elara Caring's mission and values.

• Current, unrestricted Registered Nurse (RN) license in the state of practice.
• Minimum of one (1) year of nursing experience in an acute care, skilled nursing, or clinical care setting.
• Previous home health experience is preferred, but not required.
• Experience with HomeCare HomeBase (HCHB) is a plus.
• Strong assessment, communication, and critical thinking skills.
• Reliable transportation, valid driver's license, and current automobile insurance.
• Ability and willingness to travel throughout the assigned service area.
• Physical Requirements: Sit, stand, bend, lift, and move intermittently and be able to lift 50-100 lbs.
