Job Title: Case Manager RN- Home Health Care Full-time or Part-time available
Job Location: Wilkes-Barre, PA; Pittston, PA; and surrounding areas for Luzerne County

Job responsibilities include, but not limited to the following:
  • Admits patients to Traditional Home Health Care.
  • Assesses patient status and develops plan of care in collaboration with physician, nursing supervisor, patient and family members.
  • Administers patient care, management and evaluation of the care plan, teaching of the patient in accordance with physician orders.
  • Reports patient status changes, care, and progress to physician and nursing supervisor on a continuous basis. Revises plan of care by consultation with physician.
  • Timely and accurately prepares, maintains and submits meticulous documentation as required by Traditional Home Health Care including all interactions with patients, case conferences, medication changes, various OASIS time points, and changes to plan of care.
  • Acts as a liaison and maintains contact with patients, therapist, nursing supervisor, and other members of the health care team.
  • Informs patients of other services that are available and makes referrals to other community resources as needed.

Requirements:
  • Current license as a RN and or LPN in Pennsylvania
  • Must have current driver's license
  • Ability to travel to case assignments
  • Excellent assessment skills, documentation skills and ability to meet deadlines
  • Ability to work independently with minimal supervision and manage time effectively
  • Strong interpersonal skills.
  • Demonstrates good verbal and written communication, and organizational skills
  • Acute care and/or home health experience is preferred.
