About the Role:
Project New Generation LLC is looking for a dedicated and experienced Home Health Service Director RN to lead and elevate our home health operations in Glendale, AZ. This is an exciting opportunity for a passionate nursing leader to make a meaningful impact on patient care while driving clinical excellence in a growing organization. Responsibilities:
  • Oversee and manage day-to-day clinical operations of the home health agency
  • Ensure compliance with state, federal, and Medicare/Medicaid regulations and standards
  • Supervise, mentor, and support clinical staff including RNs, LPNs, and home health aides
  • Conduct patient assessments, care plan development, and ongoing case management oversight
  • Monitor and maintain quality assurance and performance improvement initiatives
  • Collaborate with physicians, patients, and families to coordinate comprehensive care plans
  • Manage staffing, scheduling, and clinical documentation to ensure regulatory compliance
