Responsibilities:
  • Provide skilled nursing care to patients in their homes across the Northern Virginia area
  • Conduct comprehensive patient assessments and develop individualized care plans
  • Administer medications, wound care, IV therapy, and other clinical treatments as ordered
  • Monitor and document patient progress, vital signs, and changes in condition
  • Educate patients and family members on disease management, medications, and home care techniques
  • Collaborate with physicians, therapists, and care team members to ensure coordinated patient care
  • Maintain accurate and timely clinical documentation in compliance with regulatory standards
Requirements:
  • Active RN license in the state of Virginia
  • Oasis SOC Experience preferred and a plus.
  • Previous home health, med-surg, or community nursing experience preferred
  • Strong clinical assessment and critical thinking skills
  • Excellent communication and patient education abilities
  • Reliable transportation and valid driver's license for travel throughout the NOVA region
  • Ability to work independently and manage a flexible patient caseload
