Overview
The Direct Support Professional (DSP) provides personal care and support services in alignment with each Individual Support Plan (ISP). DSPs help individuals maintain comfort, build independence, and achieve an everyday life within their community. A valid driver's license and reliable vehicle are required for this role.
Key Responsibilities
  • Assist individuals with daily living and community integration skills using Person-Centered Practices.
  • Support ISP goals through training and engagement.
  • Provide personal care, including bathing, grooming, dressing, toileting, and hygiene.
  • Assist with home-living tasks such as meal preparation, laundry, light housekeeping, and shopping.
  • Monitor and report changes in condition or needs to the Executive Director or designee.
  • Complete documentation and daily progress notes accurately and on time.
  • Maintain safety standards, infection control practices, and proper equipment use.
  • Communicate effectively with team members and maintain confidentiality.
  • Attend required training and compliance programs.
  • Perform other duties as assigned.
