We are seeking a compassionate and motivated Registered Nurse (RN) to join our Behavioral Health/Counseling Specialty Clinic. This role is ideal for nurses who are passionate about patient-centered care and supporting individuals throughout their behavioral health journey.
You'll assess and triage patient needs, coordinate care, provide education, and support patients receiving behavioral health services. Working alongside counselors, behavioral health providers, and a collaborative clinical team, you'll play an important role in promoting patient safety, well-being, and continuity of care.

Responsibilities
  • Evaluate incoming patient calls and clinical concerns to assess symptoms, determine urgency, and identify appropriate next steps.
  • Provide telephone triage and guide patients to the appropriate level of care or follow-up for behavioral health concerns.
  • Coordinate care for patients with acute or complex behavioral health needs and communicate concerns promptly to the clinical team.
  • Collaborate with counselors, behavioral health providers, and other healthcare professionals to support timely, coordinated patient care.
  • Educate patients and families on treatment plans, medications, symptom management, and available behavioral health resources.
  • Provide ongoing support and resources tailored to each patient's individual needs and care plan.
  • Accurately document patient interactions, assessments, interventions, and care plans in the electronic health record (EHR).
  • Track patient needs and coordinate follow-up care to promote continuity, safety, and positive outcomes.

Qualifications
  • Registered Nurse (RN) - Active/unencumbered RN license in state of practice or compact license including state of practice.
  • All levels of RN experience will be considered. Prior experience in behavioral health, mental health, counseling, or specialty care is a plus, but not required.
  • Associate Degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) preferred.
  • For positions within UnityPoint Clinic, team members may be required to work at or float to other locations to support optimized staffing.
