As a Home Health Registered Nurse, hereโ€™s a big-picture view of what youโ€™ll do:

  • Provide home-based care to patients with illnesses, injuries and chronic diseasesโ€”often after discharge from a hospital, rehab or nursing facilityย 
  • Work with a team of physicians and oversee LPNs,ย CNAs, and aides providing patient careย 
  • Examine patients, do assessments, document progress and report changes to their physicianย 
  • Educate patients on their plan of care, prescribed medication, therapy, diet and exerciseย 
  • Coach family members on the patientโ€™s plan of care, medication and home safety ย
