Our clinical team plays a critical role in ensuring the safety, quality, and compliance of the care we deliver.

Position Summary

AbriCare is seeking a compassionate and detail-oriented Registered Nurse (RN) to join our personal care services team in the west St. Louis, Missouri area on a PRN/part-time basis for private pay clients. The RN will be responsible for conducting client assessments, developing and updating individualized care plans, performing supervisory visits, and ensuring caregivers are delivering care in accordance with each client's plan of care. This role supports both Medicaid and private pay clients and requires flexibility in scheduling.

Essential Duties and Responsibilities

  • Conduct initial and ongoing client assessments in the home setting to evaluate health status, functional abilities, and care needs.
  • Develop, implement, and update individualized plans of care in accordance with physician orders, client needs, and applicable Medicaid and private pay program requirements.
  • Perform supervisory visits of caregivers in the home as required by state regulations and AbriCare policy, including competency evaluations and skills check-offs.
  • Communicate with clients, families, physicians, and the AbriCare care coordination team regarding changes in client condition, care plan updates, and clinical concerns.
  • Ensure documentation is completed accurately and timely in accordance with AbriCare standards, Medicaid billing requirements, and state regulations.
  • Identify and report changes in client condition, safety concerns, or incidents to the Branch Director and/or Clinical leader in accordance with AbriCare's incident reporting policy.
  • Maintain compliance with HIPAA, abuse/neglect/exploitation (ANE) reporting obligations, and all applicable state and federal regulations governing personal care services.
  • Participate in quality assurance activities, care conferences, and clinical team meetings as requested.
  • Support onboarding of new caregivers through competency testing and skills validation as assigned.

Qualifications

  • Current, active, and unrestricted Registered Nurse (RN) license in the State of Missouri — required.
  • Home health, personal care, or community health experience strongly preferred.
  • Knowledge of Missouri Medicaid (MO HealthNet) personal care services program requirements preferred.
  • Current CPR/BLS certification — required.
  • Ability to pass a Level 2 background check — required.
  • Valid driver's license and reliable transportation — required.
  • Strong written and verbal communication skills.
  • Ability to work independently, manage time effectively, and prioritize in a fast-paced home health environment.
  • Proficiency with electronic documentation systems preferred.

Schedule & Compensation

  • PRN with flexible daytime hours (primarily M–F), starting light and building as client volume grows.
  • Competitive hourly rate commensurate with experience.
  • Mileage reimbursement for client visits.
  • PRN employees are not eligible for AbriCare benefits programs.

Physical Requirements

  • Ability to travel to client homes throughout the west St. Louis, Missouri service area.
  • Ability to stand, walk, bend, and lift up to 25 pounds as needed during client visits and competency evaluations.
