Position Summary
We are seeking a compassionate and dependable Registered Nurse (RN) Case Manager to provide skilled nursing services in patients’ homes in accordance with the Connecticut Nurse Practice Act, agency policies, and professional standards of practice.
The RN Case Manager is responsible for coordinating patient care, completing assessments, developing and managing care plans, communicating with the interdisciplinary care team, and ensuring high-quality patient outcomes. This role may also include supervision of other home care staff. 
Qualifications
  •  Graduate of an accredited school of nursing (BSN preferred) 
  •  Current Connecticut RN license 
  •  Minimum 1 year of acute care or equivalent nursing experience preferred 
  •  Valid driver’s license, reliable transportation, and active auto insurance 
  •  Strong communication, organizational, and interpersonal skills 
Responsibilities
  •  Perform comprehensive patient assessments, including physical, psychosocial, and environmental evaluations 
  •  Complete OASIS assessments and documentation within required timeframes 
  •  Develop, implement, and update individualized Plans of Care 
  •  Coordinate care with physicians, caregivers, and interdisciplinary team members 
  •  Monitor patient progress and report significant changes promptly 
  •  Administer medications and provide patient/family education 
  •  Maintain timely, accurate, and compliant clinical documentation 
  •  Supervise applicable home care staff, including aides and LPNs 
  •  Promote patient safety and infection control practices in the home setting 
  •  Participate in team meetings, ongoing education, and professional development 
  •  Assist with chart reviews and quality assurance activities as needed 
  •  Provide on-call coverage as assigned 
Physical Requirements
  •  Ability to sit, stand, bend, pivot, and move frequently throughout the day 
  •  Ability to lift, push, pull, and carry 20–50 lbs 
  •  Ability to travel frequently between patient homes and agency locations 
  •  Ability to use clinical equipment, laptop, and telephone systems
