Flu Vaccine Considerations

Proof of annual flu vaccination is required for all employees.

Physicians Alliance of Connecticut (PACT, LLC) is a multi-specialty Physician groups. We offer health benefits, paid time off, and a friendly working environment. We are a medium sized company with a family atmosphere.

An Internal Medical Group has an opportunity for an Advanced Practice Registered Nurse (APRN) or a Physician Assistant (PA). The position is located in West Haven, New Haven, Milford, Guilford and Wallingford and will float between locations as needed.

The Nurse Practitioner or Physician Assistant will provide services to patients in an office location. They will participate as an integral member of the healthcare team in a dynamic practice with physicians who value the role of the APRN or PA.

Essential Function

  • Provide longitudinal and urgent care to patients in an office location
  • Develop an adult and/or geriatric based patient panel
  • Educate the patients on benefits of wellness visits vs. physicals
  • Create a care plan for the patient
  • Utilize EHR, Epic to update patient chart
  • Engages patients and their families or caregivers in understanding, setting and monitoring self-management care plans, and documents individualized care plans and care coordination in the patient chart.
  • Analyze patients medical history, medication allergies, physical condition and examination to determine best care
  • Diagnose medical condition
  • Prescribes pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions.
  • Directs and coordinates the patient care activities of nursing and support staff as required.
  • Follows established departmental policies, procedures, and objectives, continuous quality improvement objectives , and safety, environmental, and/or infection control standards
  • Complete Urgent Care and Annual Wellness Visits

Education and Experience

APRN

  • The Advanced Practice Registered Nurse is masters or doctorate prepared
  • Certified Nationally as a Nurse Practitioner through AANP or ANCC .
  • Minimum of 2 years experience as a APRN required
  • The Nurse Practitioner must have possession of a valid (Connecticut) license as a Registered Nurse and an Advanced Practice Registered Nurse, and be certified as a y) Nurse Practitioner by an approved certifying body.
  • DEA, and Ct controlled substance license
  • Experience working with EHR

PA

  • Physician Assistant Certification
  • State License
  • Minimum of 2 years experience as a PA required
  • Experience working with EHR
