Job Description
Saint Luke's North Hospital is seeking a part-time Social Worker to join our team!
Responsible and accountable for assessment of psychological, social, emotional, cultural and financial needs, and the resulting treatment that impacts patients' health and recovery. Provides professional social work services that utilize clinical knowledge to address the needs of patients and families during illness, crisis intervention, and post-acute discharge planning. Collaborates with multidisciplinary team to develop transition plan and address needs in a timely manner. Participates in program planning and development to meet the needs of customers within the entity, health system and community.
Shift: Part time-16 hours per week, Thursday and Friday
The Work:
  • Makes quality patient outcomes a priority.
  • Covers OB/NICU and general medical floor
  • Overcomes obstacles to delivery of care through interdisciplinary coordination and communication.
  • Coordinates the interdisciplinary team and partners with physicians to ensure patient treatment plan progress.
  • Ensures that an appropriate plan of care is initiated.
  • Monitors care progression to ensure quality outcomes and effective transitions of care.
  • Master's in Social Work required
  • LMSW in MO required
