Part Time Registered Nurse Case ManagerAnticipated Territory: Solano County AreaSchedule: Must be available Full Days Every Saturday and Sunday (4 SOCs/day) Also willing to consider Full Time: Saturday and Sunday plus 3 week days.

What you'll do:

Utilize your outstanding clinical skills as you complete the initial assessment of patients to determine home care needs including: Conduct Start of Care (SOC) Admissions - OASISConduct follow ups, evaluation visits as needed in designated territoryMake home visits to patients in designated territoryPerform assigned duties, including reconciliation of medication, wound care, treatments, and proceduresMonitor clients' conditions, reporting changes to Clinical teamCollaborate with physicians and agency staff to deliver the best care possibleInitiate and Administer IV medication (antibiotics and Diuretics) infusions safely and effectively in accordance with established protocols.

Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.
