Position Overview:

PAY IS BASED ON PATIENTS SEEN (PER VISIT), NOT AN HOURLY RATE.

American Home Visiting Physicians is seeking Medical Doctor(MD/DO) and Nurse Practitioner(NP) to join our growing practice. This role involves providing comprehensive primary care services through in-home patient visits, including medical assessments, diagnosis, treatment planning, chronic disease management, and preventive care. The ideal candidate is committed to delivering high-quality, patient-centered care while working independently in a home-based healthcare setting.The ideal candidate is compassionate, dependable, and committed to providing exceptional patient-centered care in a home-based setting.

Responsibilities:

• Conduct comprehensive medical assessments and physical examinations in patients' homes.

• Diagnose, treat, and manage acute and chronic medical conditions.

• Develop, implement, and monitor individualized treatment plans.

• Prescribe medications and order diagnostic tests as appropriate.

• Perform routine wellness visits and preventive care services.

• Educate patients and caregivers regarding treatment plans, medications, and disease management.

• Coordinate care with specialists, caregivers, family members, and other healthcare professionals.

• Maintain accurate and timely documentation in GlaceEMR.

• Ensure compliance with all applicable healthcare regulations and clinical standards.

• Travel to patient homes within assigned service areas.

Qualifications:

Medical Doctor (MD/DO):

• Active and unrestricted Michigan Medical License.

• DEA registration or eligibility to obtain.

• Board Certification or Board Eligibility preferred.

Nurse Practitioner (NP):

• Active and unrestricted Michigan Nurse Practitioner License.

• National Board Certification preferred.

• DEA registration or eligibility to obtain.

General Requirements:

• Current BLS/CPR certification.

• Strong clinical, diagnostic, and decision-making skills.

• Experience in primary care, geriatrics, home health, or chronic disease management preferred.

• Excellent communication and interpersonal skills.

• Valid driver's license and reliable transportation.

• Ability to work independently and manage patient care effectively.

Schedule & Compensation:

• Part-Time position with potential to transition into a Full-Time role based on patient volume and caseload.
