Job Description:

The In-Patient Palliative Care Unit at Trillium Health Partners (THP), located at the Mississauga Hospital (MH) site, consists of 16 beds dedicated to providing specialized acute palliative and end of life care. 

Working in close collaboration with palliative physicians and an interprofessional team, the Registered Practical Nurse (RPN) plays a role in assessing and addressing a broad range of patient needs, along side the RN. This includes symptom management, medication concerns, psychosocial and caregiver challenges, functional changes, goals-of-care planning, and community care considerations. The RN applies specialized palliative care knowledge, critical thinking, and sound clinical judgment to identify patient needs, support safe and effective care planning, and escalate urgent clinical issues when necessary. 

Reporting to the Clinical Manager, the ideal candidate is committed to delivering exceptional patient-centered care in a dynamic environment. This role offers the opportunity to practice with compassion, excellence, and courage while supporting patients and families through complex and sensitive stages of care.

Required Qualifications: 

  • Graduate from an accredited School of Nursing program, with added courses in medication administration, phlebotomy and health assessment. 
  • Registered in good standing with the College of Nurses of Ontario (CNO) or currently meet the eligibility criteria to register in good standing in the Temporary Class or General Class with the CNO 
  • Able to function to the full scope of practice as per CNO 
  • Basic Life Support certification 
  • Computer skills including typing, sending and receiving email and working with Microsoft applications such as Word 
  • Demonstrated knowledge, experience and the ability to care for patients with the following care needs: initiation and maintenance of IV therapy, administration of medication above and below the drip chamber, blood and blood product administration, patient controlled analgesia, Enteral feeds, TPN, central venous access devices CVADS (PICCs), blood glucose monitoring, naso-gastric tubes, wound management skills, routine infection control practices, normal and abnormal blood values, oxygen therapy, tracheostomies, and chest tubes. 
  • Satisfactory attendance and performance record required 

Preferred Qualifications: 

  • Minimum two years of recent nursing experience required, preferably with palliative care, oncology, hospice, and/or complex chronic disease management 
  • Preference will be given to candidates with recent clinical experience, knowledge, education, or certification in palliative care. 
  • Canadian Nurses Association certification in Hospice Palliative Care Nursing, CHPCN(C) 
  •  Experience in providing and delivering patients and families with educational teaching 
  • Demonstrated knowledge of palliative and end-of-life care principles, including pain and symptom assessment, medication support, psychosocial needs, caregiver support, advance care planning, and goals-of-care conversations. 
  • Demonstrates effective interpersonal, communication, organizational and problem-solving skills 
  • Demonstrates patient and family-centred care values 
  • Evidence of on-going professional development 
  • Experience with patient centred and partnership care models 
  • Reliable and flexible individual who is willing to work collaboratively within an interdisciplinary team environment
