Job Summary:
We are seeking a Registered Practical Nurse (RPN) with a heart for community nursing to join our team in the GTA area. This part-time, flexible role involves providing in-home nursing care, creating individualized care plans, and ensuring clients’ well-being. The ideal candidate is kind, patient, detail-oriented, and passionate about delivering exceptional care.

Responsibilities:

  • Assess clients’ health needs and develop personalized care plans for the PSW's to follow.

  • Provide holistic nursing care in clients' homes, ensuring comfort and safety.

  • Monitor and evaluate clients' conditions, adjusting care plans as needed.

  • Maintain accurate and timely documentation of client care and progress.

  • Communicate effectively with clients, families, and other healthcare professionals.

  • Advocate for clients by promoting their rights and well-being.

  • Participate in community outreach as needed to build trust and align with our value of giving back to the community.

  • Ensure compliance with health and safety standards.

Qualifications:

  • Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario (CNO).

  • Minimum of one (1) year of recent experience in community nursing.

  • Strong problem-solving and time-management skills.

  • Excellent interpersonal and communication skills, both verbal and written.

  • Proficiency in documenting and reporting client care.

  • Valid driver’s license and reliable vehicle (preferred but not required).

  • Second language is considered an asset.
