Works as core part of our property management team to support and enhance the quality of life at a 254-unit senior property in Detroit, Michigan across Connected Communities' six key outcome areas: Stable Housing, Employment, Education, Health, Community Engagement and Economic Mobility, with a primary focus on Health.
WinnCompanies is looking for a Part-Time Temporary Community Coordinator II to join our team at Village Center, a 254-unit senior, affordable housing community located in Detroit, MI.
The Community Coordinator II is responsible for championing an innovative, resident-centered preventative health screening and wraparound services program, driving positive change and wellbeing within the community. The Community Coordinator II will play a crucial role in facilitating the delivery of on-site telehealth services to our residents on a weekly basis and building awareness of and participation in the program. Works with residents, partners, and the property management team to identify needs, interests and opportunities for individuals and the community at large, focusing on healthcare and community engagement. Efforts will help expand accessibility to crucial preventative health services, ultimately enhancing health and wellbeing outcomes for residents.
Please note that this is a temporary, part-time position that is not eligible for Winn benefits and is scheduled to work 10 hours per week. This assignment is expected to last approximately 46 weeks.
The pay for this role is $22.00-$24.00 per hour.
Responsibilities:
  • Along with external Telehealth Intervention Programs for Seniors (TIPS) team, create and implement a plan for program development and implementation of weekly preventative health screenings including resident outreach and identifying community partners and resident leaders.
  • Manage budget for health-related programming.
  • Review and track goals with Property Manager, Community Coordinator and Regional Team Lead.
  • Provide regular program updates to TIPS project management team.
  • Act as primary contact for the telehealth program, including acting as backup for the Telehealth Technician Assistant.
  • Provide direct resident services assistance, program referrals, and 1-1 coaching to individuals and families in partnership with the Community Coordinator.
  • Establish and maintain relationships with local service providers, resident leadership, community stakeholders, and other community partners.
  • Utilizing Apricot 360, collect, manage, report on, and analyze resident data, and ensure accuracy and consistency with established data.
  • Data is collected through Patient Activation Measure surveys, resident touchpoints, community programs, activities, and reporting from telehealth service providers.
  • Collaborate with Community Coordinator to effectively develop methods of communication with residents, including but not limited to identifying resident ambassadors, quarterly community meetings, a monthly community newsletter, event flyers, and social media updates.
  • Actively participate in professional development opportunities provided by the region, department, and Winn.
  • Other duties as assigned, particularly in support of property operations.

Requirements:
  • High school diploma or GED equivalent.
  • 3-5 years of related work experience.
  • Advanced skills with Microsoft applications which, include Outlook, Word, Excel, PowerPoint or Access and other web based applications.
  • Produce complex documents, perform analysis and maintain databases.
  • Ability to communicate clearly and effectively in varied written formats to internal and external customers.
  • Provide a high level of customer service to meet customer service standards and expectations for the assigned responsibilities.
  • Demonstrated success with partner management.
  • Ability to act independently in situations such as the following: arrange meetings with partner organizations, communicate schedule changes with colleagues, develop annual service plan based on community needs, reach out to appropriate person when in need of additional training or context.

  • Bachelor's degree in Social Work, Nutrition, Business, or related field.
