Job description

AMI-Wellness Home Health is seeking an experienced RN to provide part-time skilled nursing visits for home health patients in the Denver metro area.

This is a per-visit position, generally scheduled Monday through Friday based on patient needs, referral volume, and nurse availability. This is not a full-time position.

The RN will support home health patients through Starts of Care, Resumptions of Care, Readmissions, and skilled nursing revisits as assigned.

Responsibilities
  • Complete skilled nursing visits in patient homes
  • Perform Starts of Care, Resumptions of Care, Readmissions, and follow-up visits as assigned
  • Assess patient condition and communicate changes appropriately
  • Provide patient education, medication review, wound care/assessment, and other skilled nursing services within RN scope of practice
  • Communicate with physicians, supervisors, patients, families, caregivers, and the care team
  • Complete timely and accurate electronic documentation
  • Support safe transitions into home health care
Schedule and Compensation
  • Part-time, per-visit position
  • Visits generally completed Monday through Friday
  • Paid per visit, with rates varying by visit type

Requirements
  • Active Colorado RN license
  • Home health experience preferred
