Part time Registered Nurse Case Manager
Anticipated Territory : Sonoma County
Schedule: Every Saturday and Sunday Full Days. Would consider Every Saturday and Sunday plus 3 week days.

Utilize your outstanding clinical skills as you complete the initial assessment of patients to determine home care needs including:

To be a success:
  • Current and unrestricted CA RN license
  • Current BLS/CPR Certification
  • Valid CA Driver's License
  • Experience conducting assessments and Start of Care (SOC) - OASIS is highly desired
  • High level of clinical knowledge in home health care is highly preferred
  • Experience with high acuity patients is a plus
  • Experience working with an interdisciplinary team is highly desired
  • Case Management fundamentals is highly desired
  • Previous experience with IV Medication infusions highly preferred
  • Proficiencies initiating intravenous (IV) therapy is a plus
