What You'll Do

As a Home Health RN, you'll provide skilled nursing services to patients in their homes while coordinating care with physicians, therapists, nursing staff, and other members of the interdisciplinary team.

Responsibilities may include:

  • Performing skilled nursing home visits

  • Completing comprehensive patient assessments

  • Completing Start of Care and OASIS assessments as assigned

  • Developing and updating individualized plans of care

  • Monitoring patients for changes in condition

  • Providing wound care and other ordered skilled nursing services

  • Providing medication management and patient education within RN scope

  • Educating patients and family members about their care

  • Communicating with physicians regarding patient needs and changes in condition

  • Coordinating with PT, OT, LPNs, and other healthcare professionals

  • Completing accurate and timely clinical documentation

  • Completing recertifications, resumptions of care, reassessments, and discharges as assigned

  • Supporting Medicare and home health regulatory compliance

Who We're Looking For

We're looking for an RN who enjoys the independence of home health while still valuing being part of a collaborative clinical team.

Qualifications
  • Current and unrestricted New Mexico Registered Nurse (RN) license

  • Valid driver's license

  • Reliable transportation

  • Current automobile insurance

  • Ability to travel throughout Albuquerque and Rio Rancho

  • Strong clinical assessment and communication skills

  • Ability to work independently and manage a flexible visit schedule

  • Home health experience preferred

  • OASIS and Start of Care experience strongly preferred

  • Medicare-certified home health experience is a plus
