Job description

Speech Language Pathologist (SLP) – Home Health Services

Sign-On Bonus: $5,000

Employment Type: Full-time, part-time, or per diem options available

Locations: Duval/Clay and St. John's Counties

Job Purpose:

The Speech Language Pathologist will deliver evaluative, consultative, and therapeutic interventions to home health patients referred by licensed physicians in their residences. Effective communication with Home Health partners, United Rehab partners, patients, their families, and other relevant stakeholders is essential.

Key Responsibilities:
  • Conduct evaluations, assessments, and treatments in line with policy and personalized care plans, adhering to established timelines.
  • Ensure continuity of care by collaborating with the interdisciplinary team and attending physician to develop, integrate, and manage patient care plans, making referrals to other disciplines as necessary.
  • Exhibit knowledge of home care practices and demonstrate competency in specific patient care skills required for effective service delivery.
  • Identify and respond to patient care priorities with initiative and sound judgment.
  • Assist in training and orienting new team members.
  • Utilize approved educational materials for patient and family education.
  • Report any suspected violations to supervisors or relevant parties until resolved.
  • Adhere to the corporate compliance program.
  • Meet agency productivity standards while using time and resources efficiently.
  • Participate in center/agency surveys, mandatory in-services, case conferences, staff meetings, and programs such as the Performance Improvement Program as required.
  • Follow established safety protocols and report any job-related tasks that may involve occupational hazards.
  • Maintain current licensure, certification, and clinical records to comply with federal, state, and agency regulations.
  • Demonstrate self-direction, effective time management, and the ability to meet outcomes and strategic goals by set deadlines.

Licensure, Certification, and Education Requirements:
  • A valid professional license in the state of practice is required.
  • Graduation from an ASHA-accredited program. For foreign-trained candidates, the program must be recognized as substantially equivalent to U.S. entry-level education in Speech Language Pathology by an ASHA-approved credential evaluation organization.
Preferred Skills and Experience:
  • Experience in Home Health is preferred.
