Are you a compassionate Enrolled Nurse who is passionate about supporting people to live healthier, more independent lives? Join our Community Chronic Disease Management team and play a vital role in delivering high-quality, person-centred care across the Central Coast.


About the Opportunity

As an Enrolled Nurse, you will work under the direction and supervision of a Registered Nurse to deliver safe, evidence-based nursing care to patients living with chronic and complex health conditions.

This position plays a key role in supporting patients to manage their chronic health conditions while promoting independence, wellbeing and improved health outcomes. In this role you will:

  • Deliver person-centred nursing care to individuals living with chronic and complex health conditions in the community.
  • Assess, monitor and document patients' health while administering medications within your scope of practice.
  • Collaborate with Registered Nurses and the multidisciplinary team to achieve positive patient outcomes.
  • Empower patients to manage their health through education, support and evidence-based care.

About You

We are looking for someone who has:

  • Current AHPRA registration as an Enrolled Nurse (Division 2), with an approved qualification for medication administration, including intravenous medications.
  • Demonstrated clinical experience and nursing skills relevant to chronic disease management or community nursing.
  • Excellent communication and interpersonal skills, with the ability to build positive relationships with patients, carers and multidisciplinary teams.
  • Strong organisational skills, with the ability to prioritise workload and deliver safe, high-quality patient care in a dynamic environment.
  • A commitment to evidence-based practice, continuous improvement and person-centred care, along with a current valid Australian Driver Licence.
