About Us -

The Case Management team initiates discharge planning with every patient upon admission and provides assistance with aftercare, medical equipment, care coordination, and other discharge needs. Case managers are assigned to individual units but also may follow patients with specific discharge needs to provide targeted, patient-centered care. The Case Management team collaborates with physicians, nurses, ancillary staff, patients, and families to coordinate excellent care. Case Managers are critical members of the multidisciplinary team and maintain consistent communication with care providers. The Case Management team strives to achieve timely discharge of patients with all needed aftercare arranged.

What You'll Do

Provide behavioral health/safety risk assessments in the Emergency Department and coordinate aftercare. Ensure safety of patients by assigning risk level. Assist providers, police officers, and outside entities by ensuring validity of involuntary hold paperwork and filing involuntary holds with the appropriate counties.

Requirements

Licensed Social Worker, Therapist or Social Services Provider (BSW) OR four year degree in a related field.
