Job description

Shift: Saturday and Sunday every other weekend (7p–11p) (Part-Time) (1 opening)

Peachtree Hills Place is seeking an exceptional Care Partner (Certified Nursing Assistant) to join our Assisted Living Household. This role is vital in helping Members maintain independence, dignity, and joy while receiving warm, person-directed care. You will play a key part in creating meaningful daily experiences and supporting Members in living life to the fullest.

GENERAL SUMMARY:

The Care Partner provides compassionate, person-directed support that honors each Member’s unique needs, abilities, and daily preferences. In alignment with our Model of Care, the Care Partner assists with personal services, daily living activities, and social, emotional, and cognitive engagement. This position also includes limited health maintenance tasks and fosters an atmosphere where Members feel connected, respected, and empowered.

ESSENTIAL JOB DUTIES:

Assist Members with dressing, grooming, bathing, eating, toileting, positioning, turning, and transferring.

Respond promptly and warmly to call lights to ensure Member comfort and safety.

Escort, transport, and support Members during mobility needs.

Maintain clean and organized Member rooms: change linens, make beds, manage laundry, and ensure neatness.

Provide the highest level of person-directed care while honoring Member preferences and choices.

Document observations, care provided, and any incidents accurately by the end of the shift.

Support the Model of Care by working collaboratively with team members to ensure consistent, dignified, and personalized service.

Other Duties:

Maintain safe, sanitary, and orderly workspaces, equipment, and household areas.

Encourage Member independence and facilitate meaningful engagement through activities, conversation, and one-on-one interaction.

Attend in-service training, required educational sessions, and team development opportunities.

Perform additional duties and responsibilities as assigned by the Household Guide.

QUALIFICATIONS:

Successful completion of a State-approved geriatric nursing assistant training program.

Active registration on the State CNA registry.

Previous geriatric care or assisted living experience preferred.

Ability to read, write, and follow instructions in English.

Strong communication and documentation skills.

PHYSICAL REQUIREMENTS/WORKING CONDITIONS:

Frequent standing, stooping, bending, stretching, and squatting required.

Ability to stand or walk for up to 80% of the workday.

Must be able to assist with lifting, transferring, turning, and positioning Members (including support for Members weighing over 125 lbs.).

Ability to lift and carry 20 lbs. frequently and up to 50 lbs. occasionally.

May encounter offensive odors, behavioral challenges, and exposure to communicable diseases (including HIV and HBV).

Must maintain focus, attention to detail, and professionalism amid frequent interruptions.
