What You Will Do as a Home Health RN Filed Nurse

As a Home Health RN Filed Nurse you will You will build from resources of the community to plan and direct services to meet the needs of patients and families within their homes and communities.Some of your primary responsibilities will include:

  • Completing an initial assessment of patient and family to determine home care needs. Providing a complete physical assessment and history of current and previous illness(es)

  • Supervising and directing the Licensed Practical Nurse (LPN) and/or the Home Health Aide in implementing the patient care plan

  • Developing patient education plans, as needed, and directing the LPN in implementing said plans

  • Creating and modifying the Home Health Aide care plan and supervising them via patient feedback and onsite interactions

  • Evaluating and revising plan of care as patient status and needs change

  • Using health assessment data to determine nursing diagnosis

  • Developing a plan of care, which establishes goals based on nursing diagnosis and incorporating therapeutic, preventive, and rehabilitative action.

  • Administering medications and treatments as prescribed by the physician

  • Identifying discharge planning needs as part of the care plan development and implementing prior to discharge of the patient

  • Acting as the Case Manager when assigned by the Clinical Supervisor and assuming responsibility to coordinate patient care for assigned caseload

  • Preparing clinical notes as needed and communicating with the physician regarding patient's needs

Education, Licensure & Certification Requirements

  • Graduate of an accredited school of nursing.

  • Active RN licensure

  • One (1) to two (2) years of recent acute care experience in an institutional setting

  • Bachelor's degree with one (1) year of home health care experience preferred

  • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
