Modena Green Oaks, our beautiful Assisted Living and Memory Care community in Lake Bluff, Illinois, is seeking a detail-oriented Registered Nurse (RN) Assessment Consultant to support the accuracy, integrity, and regulatory compliance of resident assessments and service plans.

This is an ideal opportunity for an experienced Illinois RN who enjoys clinical documentation, regulatory compliance, and quality assurance. This is not a direct care or supervisory position. Instead, you'll serve as a trusted clinical resource, ensuring resident assessments are complete, compliant, and survey-ready while partnering closely with the Executive Director and Director of Nursing.


About the Role

As our RN Assessment Consultant, you will review and validate resident assessment documentation and ensure it is properly reflected in each residentโ€™s service plan, in accordance with the Illinois Assisted Living and Shared Housing Act (210 ILCS 9) and its implementing regulations (77 Ill. Adm. Code 295), as well as the Illinois Alzheimerโ€™s Disease and Related Dementias Special Care Disclosure Act. Your focus is documentation integrity and regulatory readiness โ€” supporting the communityโ€™s compliance posture during Illinois Department of Public Health (IDPH) surveys and day-to-day operations.


What Youโ€™ll Do

  • Review resident assessment documentation โ€” including initial, annual, and change-of-condition assessments โ€” to confirm it is complete, accurate, and consistent with Illinois assisted living requirements (77 Ill. Adm. Code 295).

  • Review and cosign negotiated service agreements (NSAs) / service plans to ensure assessed care needs are accurately reflected in each residentโ€™s plan of care.

  • Support Memory Care assessment documentation as required under the Illinois Alzheimerโ€™s Special Care Disclosure Act.

  • Partner with the Director of Nursing (DON) to translate assessment findings into appropriate, up-to-date care plan updates.

  • Review assessments flagged by the DON for changes in condition, hospital returns, or significant clinical events.

  • Maintain documentation standards that meet IDPH survey requirements and support the community during inspections and any plan-of-correction follow-up.

  • Participate in QA meetings or care conferences as requested by the ED or DON, on an as-needed basis.


What You Bring

  • Current, unrestricted Illinois RN license in good standing with IDFPR (Illinois Department of Financial and Professional Regulation).

  • Minimum 2 years of nursing experience in assisted living, memory care, skilled nursing, or a related long-term care setting.

  • Working knowledge of 77 Ill. Adm. Code 295 and Illinois Memory Care disclosure requirements.

  • Familiarity with IDPH survey processes and plan-of-correction standards.

  • Strong documentation skills with a sharp eye for regulatory accuracy.

  • Ability to work independently and communicate effectively with the ED and DON.

Nice to Have
  • Prior experience in a combined assisted living and memory care community.

  • Experience supporting communities through IDPH licensure surveys or complaint investigations.

  • Familiarity with electronic health record (EHR) systems used in Illinois assisted living.
