Position Summary

Nurse Next Door Central Valley & Bay Area is seeking a compassionate, skilled, and dependable Licensed Vocational Nurse (LVN) to join our growing clinical team serving the Central Valley & Bay Area. The LVN will provide direct skilled nursing care in clients' homes under the supervision of the Director of Patient Care Services (Registered Nurse).

This role is ideal for an LVN who thrives in a fast-paced home health environment, values meaningful patient relationships, and is passionate about improving quality of life for seniors and medically fragile individuals.


Responsibilities
  • Provide direct skilled nursing care within LVN scope of practice
  • Implement physician orders and individualized care plans
  • Conduct ongoing patient assessments and accurately report changes in condition
  • Administer medications and treatments as prescribed
  • Document all care, observations, and interventions timely and accurately
  • Educate patients and families regarding diagnoses, medications, treatments, and disease management
  • Assist with care coordination and communication among interdisciplinary team members
  • Recognize emergencies and respond appropriately
  • Follow infection control standards and Universal Precautions
  • Maintain patient dignity, confidentiality, and rights at all times
  • Participate in performance improvement and quality assurance initiatives
  • Travel to patient homes throughout assigned service areas

Qualifications
  • Current California LVN License in good standing
  • Graduate of an accredited vocational nursing program
  • Minimum 1 year nursing experience required
  • Home health, hospice, or medical-surgical experience preferred
  • Current CPR certification
  • Strong clinical judgment and assessment skills
  • Excellent communication and customer service abilities
  • Ability to work independently with minimal supervision
  • Valid driver's license, reliable transportation, and current auto insurance
  • Negative TB test or chest X-ray

Physical Requirements
  • Ability to stand, walk, bend, squat, kneel, twist, push, and pull
  • Ability to lift up to 50 pounds
  • Visual and hearing acuity required
  • Ability to work in varying patient home environments
