PROGRAM: Forensic Intensive Recovery (FIR)

JOB OVERVIEW:

The FIR Case Manager works directly with participants who are court-stipulated for treatment services with oversight from the Adult Probation and Parole departments. The primary responsibilities are to ensure that services are provided to participants and documented according to DAP, CBH, Forensic Services, and PHMC requirements. This individual will provide support to the participants while working with all levels of management within the treatment program, Forensic Services, and the recovery community.

Reports to: FIR Case Management Supervisor

RESPONSIBILITIES:

Responsibilities include, but are not limited to, the following:

• Provide case management services to a chemically dependent forensic population, with a caseload of up to 45 active participants.
• Establish and maintain service productivity goals at minimum designated service units per week.
• Document service provision within 24 hours.
• Participate in monthly team meetings and scheduled case conferencing with Probation, meeting reporting requirements.
• Participate in biweekly supervision meetings.
• Collaborate and attend meetings with Probation/Parole, treatment providers, and other collaborating agencies.
• Complete discharge summaries within timeframes specified by the program.
• Assist individuals to link to recovery support services, including PRO-ACT recovery center, Access to Recovery, faith-based institutions, mental health services, and other identified community supports.
• Create weekly schedules detailing activities to be completed for each week.
• Maintain participant records in accordance with agency fee-for-service standards, managed care, and licensing requirements.
• Prioritize assisting participants to enroll in treatment, physical health services, DPA, vocational services, and educational services.
• Identify, coordinate, and link community resources to each participant to promote accessibility to recovery-oriented community services.
• Coordinate and communicate with other programs within and outside the agency to foster a collaborative team approach.
• Spend an average of 80% of each workweek in the field for off-site participant appointments and meetings.

• Bilingual fluency is a plus (especially Spanish/English)

QUALIFICATIONS:

• Valid driver’s license required
• Must meet at least one of the following:
  • Have an associate degree in social services, sociology, psychology, criminal justice, or social work and have 2+ years of experience in public and private human services including 1+ years of direct participant contact in an addiction recovery, mental health, and/or behavioral health setting
  • Have a bachelor’s degree with major course work in sociology, social work, psychology, anthropology, political science, history, criminal justice, counseling, or education and have 1+ years of experience involving direct participant contact in an addiction recovery, mental health, and/or behavioral health setting
• Preferred: Experience in direct case management services with a chemically dependent criminal justice population
• Preferred: Experience in fee-for-service delivery
