Job description

Job Title: Intake Coordinator PRN

Location: Prince George County

Facility Address: 3000 Lottsford Vista Road, Bowie

Weekend and Weekday Shifts Available

Deliverables/Principal Results Expected:
  • Explain and sign admission paperwork;
  • Process and copy driver's license/insurance information;
  • Create/start client treatment file.
  • Contact Probation/Parole/Employer if needed for the client and assist in obtaining a letter for admission/discharge.
  • Facilitate the admission/discharge process by communicating with all departments as needed.
  • Maintain paperwork, notes, and application statuses as required for client stay/discharge needs per funding source requirements.
  • Conduct level of care assessments on all clients.
  • Complete aftercare goals with detox-only clients, set up aftercare, and complete discharge summaries.
  • Perform other duties as assigned.
Licensure, Education, & Experience:

High school diploma or equivalent required. Associate's or bachelor's degree in a human services-related field preferred.
