Job Title: Direct Support Professional- Palmyra

DEPARTMENT: Community Resources
SUPERVISOR'S TITLE: In-Home Services Regional Manager or In-Home Services Manager/SFC
POSITIONS SUPERVISED: None
JOB STATUS: Full-time , Non-Exempt
SCHEDULE: Flexible schedule based on the needs of the individuals served; may include travel to trainings, meetings, and community or home-based appointments


Summary:
Provide individualized, in-home, and community-based support to individuals with disabilities. This role focuses on promoting independence, developing life skills, and supporting individuals in achieving personal goals while remaining in their home environment.


Key Responsibilities:
Demonstrate empathy and interest in individuals served
Assess habilitation needs and assist in the development and implementation of individual service plans
Train and support persons served in daily living skills, self-help care, communication, social skills, self-advocacy, health and safety, and community resource use
Provide services and assistance in the person served's home or in the community
Assist with incidental household tasks as needed
Escort persons served to medical appointments as necessary
Assist with correspondence and bill paying as necessary
Maintain records, reports, and documentation in compliance with established guidelines and standards
Maintain supplies and develop materials needed to support goal requirements
Recommend ancillary services as needed
Serve as a resource to families, advocates, and other involved parties
Attend team meetings and training sessions as directed
Promote positive community relations and support opportunities for growth
Participate as a member of the In-Home Services team
Maintain confidentiality of employee and person served information
Maintain a flexible work schedule to meet program needs
Participate in trainings to improve and maintain job skills
Accept supervision and perform other duties as assigned


Qualifications & Skills:
Experience working with people with disabilities
Ability to verbally communicate with co-workers and persons served
Strong written communication and documentation skills
Ability to use word processing, database, and spreadsheet software
Ability to work collaboratively and follow direction
Interest in supporting individual growth, independence, and community participation


Physical & Work Requirements:
Ability to lift and carry up to 50 pounds
Ability to stoop and bend to assist persons served
Ability to travel to trainings, meetings, and community or home-based services
Maintain dependable transportation
Valid driver's license required
Supply proof of vehicle liability insurance
Ability to transport clients as required by the position


Certifications & Training Requirements:
First Aid and CPR
Preventing Disease Transmission (PDT)
Certified Restraint Training (CRT)
Medication Administration (Core A and B)
PCISP Implementation
Person Served Specific Training
Training completed upon hire and annually, including incident reporting, person-centered planning/individual rights, abuse/neglect/exploitation, health and wellness, communication, medication administration, emergency/evacuation procedures, personnel policies, and safety during lifting/transferring
