How you'll contribute 

The Outcomes Manager coordinates collection, documentation, and submission of Inpatient Rehabilitation Facility Patient Assessment Instrument (IRF-PAI) and Quality Indicators (QIs) data in accordance with CMS requirements and facility policies. Partners with the interdisciplinary team to ensure timely, accurate data capture and supports education on QI documentation standards. Works closely with nursing, therapy, case management, and medical staff to support accurate QI scoring and IRF-PAI completion. Adheres to CMS regulations, payer guidelines, and organizational policies related to data integrity and timeliness. May assist with audits, performance improvement activities, and data validation.

Essential Functions:

  • Coordinate PPD data collection with the interdisciplinary team.
  • Ensure supportive documentation for QIs is complete in the medical record.
  • Complete admission and discharge IRF-PAIs within CMS-designated timeframes.
  • Abstract required information from medical records and summarize on the IRF-PAI.
  • Ensure admission IRF-PAI completion within four (4) days of admission.
  • Ensure discharge IRF-PAI completion within five (5) days of discharge.
  • Enter data into the CMS-accepted software/program within designated timeframes.
  • Maintain and update the IRF-PAI log; verify accuracy against the patient location roster.
  • Ensure all columns/fields of the IRF-PAI log are completed in collaboration with each discipline.
  • Assist with team education related to QIs and documentation standards; complete and help administer QI competencies for team members.
  • Perform other duties as assigned.

What we're looking for...

Education/Licensure: Clinical degree with current license to practice as an OT, PT, RN, SLP, or MSW.

Knowledge: QI competency; working knowledge of state and federal guidelines; familiarity with healthcare workflows and documentation.

Skills: Basic computer and data entry proficiency; strong clinical assessment/analysis; effective verbal and written communication; excellent interpersonal skills; ability to collaborate across disciplines and meet strict deadlines with high accuracy.
