Job description

POSITION: Police Assisted Diversion (PAD) Case Manager

DEPARTMENT: Prevention and Diversion

REPORTS TO: Director of Outreach Services


PROGRAM SUMMARY

One Day at a Time (ODAAT) operates a Police Assisted Diversion (PAD) program for ROC North, covering Northwest Philadelphia. PAD is a program offered to individuals who are stopped for a variety of criminal offenses and social referrals. Eligible criminal offenses include; non-violent drug, prostitution and retail theft offenses within the designated districts and during program operating hours, 8am-11pm. The program works to divert individuals away from the criminal justice system and towards supportive, peer-based social services that are customized to the participants' needs.

PAD is a collaboration between police officers, service providers and community residents that utilizes a health-centered approach to law enforcement. PAD at ODAAT works with engaged individuals to address issues such as access to basic needs, access to treatment services, appropriate medical care and the development and achievement of short and long-term goals. PAD services are voluntary, free to the individual and may continue for as long as the participant decides to remain engaged in program services.

The PAD team collaborates with the individual to provide a comprehensive service plan for the individual geared to reduce the risk of criminal activity. The PAD team approaches each individual with a holistic and community approach that utilizes a variety of tools including assessments/evaluations, intake procedures and coaching/case management.


POSITION SUMMARY

The PAD Case Manager is a caring and compassionate individual who performs professional and administrative duties to screen, assess, facilitate and coordinate a new unique diversion program that provides alternative resources for individuals to avoid placement in jail and/or future police related encounters. The PAD Case Manager will conduct biopsychosocial assessment, service care plans, inter-agency collaboration and referrals. The PAD Case Manager will conduct outreach and refer for adjunct services such as social services, mental health or non-treatment related services. The PAD Case Manager will provide knowledge of available resources to the PAD team to ensure diversion participants are receiving maximum benefits of the PAD program.


KEY DUTIES AND RESPONSIBILITIES

  • Coordinate the intake process for individuals, both police and social diversions
  • Perform a variety of outreach services and efforts
  • Conduct case management duties which include and are not limited to, goal setting, needs assessment, linkage to care and resources
  • Complete progress notes in DAP format and maintain a participant file for auditing to ensure compliance
  • Assists with program planning, implementation and evaluation of activities
  • Participates in community outreach and public relations activities to enhance network of resources
  • Work cooperatively and collaboratively with Philadelphia police to serve the needs of the community and participants
  • Assist with public, media and internet outreach with other staff and program volunteers to support intake into the programs and attendance at sponsored events
  • Assist with planning and supporting outreach events
  • May facilitate/co-facilitate support groups
  • Attend all mandated training activities
  • Provide programming under the Harm/Risk reduction and Trauma-informed service spectrum and ODAAT services to high-risk individuals
  • Ability to organize time and prioritize assignments
  • Ability to work flexible hours
  • Actively support executives with client and partner collaboration
  • Ensures compliance with related federal, state and local regulations, as well as ODAAT/UAC and county policies and procedures
  • Adhere to all company policies related to the Driverโ€™s Standard Operating Procedures
  • Other duties as assigned


SKILLS, KNOWLEDGE AND EXPERIENCE

  • Possess ability to understand various policing and community operations and procedures
  • Able to maintain a professional, customer service-oriented attitude at all times with strong interpersonal skills
  • Ability to meet face-to-face with individuals of diverse backgrounds and disabilities, including physical, mental, substance abuse, HIV/AIDS, etc.
  • Proficient with Microsoft Office software, including Excel and Outlook
  • Demonstrated written and verbal communication skills including typing and writing
  • Must have the ability to work independently and as a team member
  • Must take initiative and ability to problem solve
  • Ability to learn and apply written policies and procedures related to work
  • Able to set and observe appropriate boundaries with clients

ABILITIES AND QUALIFICATIONS

  • Bachelor/ Associate Degree in Social Work or related field, preferred
  • 1+ years of prior outreach and community work experience in the field and/or related field preferred
  • Proficient in Microsoft Office Suite software
  • Ability to manage time and respond to deadlines
  • Strong written and verbal communications skills
  • Able to develop rapport, provide information and refer with sensitivity to cultural issues
  • Bilingual is a plus


LICENSES AND CERTIFICATIONS

  • Valid Driverโ€™s License required


PHYSICAL REQUIREMENTS

  • Able to lift/move weight up to 50lbs.
  • Must be able to sit for 2 or more hours per day.
  • Must be able to stand for 2 or more hours per day.


WORK ENVIRONMENT

The employee will work in an office and public, service-delivery environment. Hectic situations can occur in the public area of the facility characteristic to working in a community. Potential exists for volatile and unpredictable situations to occur.
