Job description

Job Title: Perm - Nurse Practitioner
Location: Greater Columbus Area, Ohio (Including Dublin, Columbus, Lancaster, Hilliard)
Schedule: Full-time | Monday–Friday

Position Summary:

We seek a compassionate and dedicated Nurse Practitioner to provide comprehensive patient care in senior care facilities across the Greater Columbus area. This is an excellent opportunity for clinicians passionate about geriatric care and chronic disease management, with strong support and mentorship provided.

Key Responsibilities:
  • Deliver direct patient care, including assessing, diagnosing, and managing acute and chronic health conditions.

  • Educate patients and families on self-care strategies for chronic conditions.

  • Order, perform, and interpret diagnostic tests to support clinical decision-making.

  • Collaborate with interdisciplinary teams within senior care facilities.

  • Maintain accurate and up-to-date patient records in EMR systems.

  • Prescribe medications and develop individualized care plans.

Qualifications:
  • Active Nurse Practitioner license in Ohio (required).

  • Minimum 6 months of clinical experience (1+ year preferred).

  • Experience in geriatrics, urgent care, or chronic disease management is a plus.

  • Proficiency in EMR systems such as DocNow or Tebra; experience with Dragon Medical One is desirable.

  • Must have reliable, insured transportation (daily travel within a 20–40 mile radius).
