About the Role:

As an RN Case Manager - Care Ally at Quincy Medical Group, you will play a vital role in helping patients navigate the healthcare system while improving health outcomes and quality of life. Working collaboratively with providers, patients, families, and community resources, you will coordinate care for patients with complex medical needs, ensuring they receive the right care at the right time.

This position is ideal for a nurse who is passionate about care coordination, patient advocacy, and population health. Through proactive case management, you will help reduce barriers to care, prevent avoidable hospitalizations, and support patients in achieving their health goals.

Schedule & Work Environment

• Full-time position, Monday-Friday, 8:00 AM - 5:00 PM.
• No weekends, holidays, or overnight shifts.
• Office-based position with collaboration across multiple clinical departments and community partners.
• Opportunity to work closely with providers, care teams, patients, and families to improve care coordination and patient outcomes.

Primary Responsibilities

• Identify patients who may benefit from case management services using predictive modeling tools, provider referrals, and clinical data.
• Conduct comprehensive assessments of patients' medical, psychosocial, environmental, and support needs.
• Develop, implement, and monitor individualized care plans that promote quality outcomes and patient-centered care.
• Coordinate services across the continuum of care, including primary care, specialty care, hospitals, home health, and community resources.
• Collaborate with providers, patients, caregivers, and interdisciplinary teams to support care transitions and reduce avoidable hospital admissions and readmissions.
• Educate patients and families regarding disease management, medications, treatment plans, preventive care, and available healthcare resources.
• Serve as a resource for providers, staff, patients, and families regarding case management services, health benefits, and community programs.
• Maintain timely, accurate documentation within the electronic medical record and complete required reporting activities.
• Monitor patient progress and adjust care plans based on changing clinical needs and patient goals.
• Participate in quality improvement initiatives, policy development, and ongoing evaluation of case management processes.
• Maintain current knowledge of case management standards, regulatory requirements, and evidence-based practices.
• Perform other duties as assigned to support Care Ally and population health initiatives.

Qualifications

• Current Illinois Registered Nurse (RN) license required.
• Graduate of an accredited school of nursing required.
• Current CPR certification required.
• Previous case management, care coordination, population health, utilization management, or chronic disease management experience preferred.
• Strong clinical assessment, critical thinking, and care coordination skills.
• Knowledge of community resources, healthcare systems, and care transition processes.
• Excellent communication, organizational, and interpersonal skills.
• Ability to manage multiple patient cases while working independently and collaboratively.
• Experience with electronic medical records and population health tools preferred.

The compensation for this role includes a base pay range of $52,000 - $78,000 annually, with actual pay determined by factors such as skills, experience, education, certifications, geographic location, and internal equity. Additional compensation may be available through bonuses and other incentives. Base pay is only a portion of the total rewards package.
