Job description

JOB TITLE: Home Health RN Case Manager (RNCM) + Recertification Nurse
  • This is a PRN-only RNCM position with our agency. We're looking for an experienced RN Case Manager interested in building a long-term employment relationship with our agency by consistently accepting and executing RNCM and recertification visits and weekly medicine setups as needs arise, and providing in-office State Survey support as needed (1-2x/year) in our Perrysburg/Toledo Office, under the guidance of our Northwest Ohio Administrator.
    • There is potential for growth into a Part-Time or Full-Time RNCM position as our agency grows over the next 6-12 months, though we cannot guarantee additional hours at this time.
    • Contractors / Contracted Arrangements will also be considered.
    • RNs with nights-only and weekends-only availability cannot be considered at this time.
  • Because of the flexible schedule, many nurses choose to pair this opportunity with other employment, and we're happy to work around existing commitments so long as you are able to accept most / all of our client care requests and support State Surveys.
  • This role requires a mix of RNCM services and recertification visits - and travel - as needed to all clients across the far-West Northwestern Ohio region.
    • This role works closely with and reports directly to our NW OH Regional Program Administrator (local) and Director of Clinical & Administrative Operations (statewide).
    • For those pursuing career growth: a defined path for advancement to Full-Time RNCM, Assistant Program Administrator, Program Administrator, Clinical Administrator, Director of Nursing (DON or Asst. DON), Business Administrator, QA / Compliance leadership, and other positions will be supported.
  • Work consists of: case management, starts of care (SOC), admissions, recertifications, transitional care and resumptions of care (ROC), clinical reviews, and other assessments and services, as well as new hire orientations, in-service and supervisory reviews, and input to influence our training and education, care practices and outcomes, and the success of our clients, employees and business overall.
  • At least 1 year of experience is required in: Case Management, Case Admissions and Discharges, wound care, PICC/PORT/IV Care, Transitional Care, lab draws, OASIS and Non-OASIS assessments, and excellent documentation and field staff leadership.
  • At least 1 year of paid experience is required at/with a home health agency or in-home care provider (or equivalent); 2-5 years' experience is preferred.
  • Desirable but not required:
    • Nurse Supervision experience
    • People management
    • Peer/staff education and development
    • Success or willingness to build relationships with client payors, case mgr / entity relationships (i.e., AAA, DoDD County Board, VA, etc.) in NW Ohio and our current + prospective employees in these communities.

EMPLOYMENT TYPE & SCHEDULE: PRN (as-needed) or Contractor
  • To start, this PRN role will consist of 1-2 consistent weekly med setups and 1-10 recertifications per week, Monday-Friday between 8:00 AM and 5:00 PM.
    • Occasional early-evening and rare Weekend visits occur but are not required.
    • Additional service requests will be added over time, built to fit your availability and travel preferences.
  • No Overnight or Overtime shifts required!
  • We prioritize visits as close to your home as possible - that fit your availability and preferences.
  • Hourly Compensation (paid weekly, on Fridays via direct deposit).

SERVICE AREA: Far-West / Northwestern Ohio region (Defiance, Paulding & Van Wert Counties) plus surrounding 1-hour radius required
  • HCN requires typical travel distances in a 10-60 minute radius, and occasional/rare travel up to 2 hours.
  • Additional hours are available with expanded travel radius if desired (not required).

Job Summary
As an RN Case Manager, you will be instrumental in coordinating and providing patient care and ensuring optimal outcomes.
  • With core skills in managed care, patient assessment, client care and treatment plan documentation, and discharge planning, you will oversee patient services and provide expert nursing care.

Responsibilities/Requirements of an RN Case Manager / Recertification Nurse include:
  • RN performs client assessments while directing, planning, and initiating appropriate actions in home care situations.
  • RN manages/supervises a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
  • RN to Document/Communicate/Coordinate patient status/change in condition to care team.
  • RN to perform job duties while promoting personal safety and a safe environment for clients and coworkers.
  • Willing to travel surrounding counties.
  • Minimum of one (1) years of nursing supervisory experience. Home care experience preferred.

Qualifications
  • Graduate of an accredited School of Nursing.
  • Minimum of one (1) year experience as a Registered Nurse, with an active RN License / ability to practice in the State of Ohio.
  • Minimum of one (1) year of experience in a home health or acute care setting or equivalent experience. 1+ years of paid home health / in-home care experience is also required. 2+ years of both are preferred.
  • Minimum of one (1) year Experience in case management is required.
  • CPR and First Aide Certifications required (existing or can be obtained prior to hire date through our agency or accredited third parties).
  • Experience in a supervisory role, wound care, lab draws, and OASIS assessments is a plus.
  • Strong background in nursing, patient care, and confidentiality.
  • Strong attendance and dependability track record with previous employers.
  • Desirable character traits: reliable, honest beyond reproach, professional, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful, and positive.
  • Familiarity with HIPAA regulations and medical terminology.
  • Current Driver's License and reliable transportation.
