Job description

JOB TITLE: Home Health RN Supervisor / RN Case Manager (RNCM)
  • Work consists of case management, starts of care (SOC), recertifications, resumptions of care (ROC), and other assessments and services, as well as occasional new hire orientations, in-service and supervisory reviews, and input to influence our training and education, care practices and outcomes, and the success of our clients, employees and business overall.
    • Requires back-up for occasional Skilled Nursing visits in most areas.
  • At least 1 year of experience is required in: Case Management, wound care, PICC/PORT/IV Care, lab draws, plus OASIS and Non-OASIS assessments. Nurse Supervision experience is a plus.
  • At least 1 year of paid experience at a home health agency (or equivalent) is required; 2-5 years' experience is preferred.

SCHEDULE TYPE & SERVICE AREA:
  • The primary service area is focused in and around Findlay, Fostoria, and Tiffin, Ohio, [including all areas of Seneca and Hancock Counties] however coverage of up to a 2-hour radius (from your home) of all surrounding areas is required.
    • Qualified applicants may reside in or near any part of the service areas so long as travel requirements are fulfilled.
  • This is a weekday position (Monday - Friday) requiring both daytime and evening availability.
    • No Weekends required.

EMPLOYMENT TYPE: Long-term employment for Full-time RNCM/Recert Nurses*
  • No On-Call requirements / No Overnight travel required.
  • Weekend services are optional when available but are not required.

To support our growth and the increasing volume of client care throughout Ohio, we are seeking a highly skilled, experienced, accountable, and self-motivated home health RN Case Manager (RNCM) / Recertification Nurse to join our agency and serve existing and returning clients throughout Northwest Ohio including Seneca and Hancock Counties and surrounding areas in a full-time capacity.

  • For those pursuing career growth: a defined path for advancement to Assistant Program Administrator, Program Administrator, Clinical Administrator, Business Administrator, QA / Compliance leadership, and other positions will be supported.
  • Services primarily include recertifications, however opportunities for starts of care (SOC), resumptions of care (ROC), and other assessments and services are available (not required), as well as new hire orientations and supervisory reviews.
  • At least 1 year of experience in home health is required, with 2-5 years highly desirable.
    • Must have experience with OASIS and NON-OASIS assessments.
    • 1 or more years of experience in wound care, lab draws, and OASIS assessments is a plus.

Compensation:
  • Full-time Hourly Wage: $35 - $40/hour
  • Full-time Annual Salary* (+ PTO) starts between $72,800 - $83,200 per year, based on 2-hour travel radius and agreed-upon average recertifications per week plus quarterly bonus opportunities available. *Requires pre-determined minimum visits/services per week.

Job Summary
As an RN Recertification Nurse, you will be instrumental in coordinating, re-evaluating and confirming the medical necessity of continued home health care services for our patients, and providing patient care and ensuring optimal outcomes.
  • With core skills in managed care, patient assessments and compliance, client care and treatment plan documentation, and discharge planning, you will oversee patient services and provide expert nursing care.
  • You will submit updated documentation of the patient's condition and ongoing care needs timely and within compliance of all state, federal, and agency guidelines.
  • You will ensure the successful communication with and between our physicians, our agency staff, service administrators, other caregivers and clinicians, and the client + his/her family.

Responsibilities/Requirements of an RN Case Manager / Clinical Supervisor include:
  • RN performs client assessments while directing, planning, and initiating appropriate actions in home care situations.
  • RN manages/supervises a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
  • RN to Document/Communicate/Coordinate patient status/change in condition to care team.
  • RN to perform job duties while promoting personal safety and a safe environment for clients and coworkers.
  • Willing to travel surrounding counties.
  • Minimum of one (1) years of nursing supervisory experience. Home care experience preferred.

Reports to: Regional Program Administrator, or Clinical Administrator/Director of Nursing
Qualifications
  • Graduate of an accredited School of Nursing.
  • Minimum of 1 year experience as a Registered Nurse, with an active RN License / ability to practice in the State of Ohio.
  • Minimum of 1 year of experience in home health agency or equivalent setting.
  • CPR and First Aide Certifications required (or can be obtained prior to hire date through our agency or accredited third parties).
  • Experience with OASIS and NON-OASIS assessments is also required.
  • Strong background in nursing, patient care, and confidentiality.
  • Exceptional on-time and attendance record, and consistent availability aligned to your chosen schedule type (PT/FT/Hybrid)
  • Desirable character traits: reliable, honest beyond reproach, professional, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful, and positive.
  • Familiarity with HIPAA regulations and medical terminology.
  • Willingness and ability to perform required physical demands (see below)
  • Consent + Ability to pass a State & Federal background check (including fingerprints)
  • 18yo + active Driver's License / State ID + US Work Authorization
  • Reliable transportation to all scheduled visits, within full reach of stated travel radius and assigned clients (up to at least 2 hours is required).
