Job Description:

We are seeking an experienced Registered Nurse Care Manager to support its Inpatient Care Management team.ย 

The ideal candidate will have a strong background in Care Coordination, Utilization Review, and Discharge Planning within an acute care hospital setting.

Key Responsibilities:

  • Coordinate inpatient care management activities.
  • Perform utilization review and ensure appropriate level of care.
  • Develop and facilitate discharge plans for patients.
  • Collaborate with interdisciplinary healthcare teams.
  • Maintain accurate documentation and care management records.
  • Support patient-centered care and resource utilization initiatives.

Must-Have Qualifications: โœ” Active RN License in the State of New York โœ” Associate Degree from an accredited Nursing Program accepted per facility requirements โœ” Minimum 5 years of clinical experience as a Registered Nurse โœ” Inpatient Care Management experience (IP experience required; ED-only experience not preferred) โœ” Experience with Care Coordination, Utilization Review, and Discharge Planning โœ” Current BLS Certification โœ” NYSED Primary Source Verification of RN License and Registration โœ” PRI & Screen Certification (must be available at submission or obtainable per facility requirements)
