Remote Triage Nurse (RN) - Overnight

Location: Minnesota - supporting six Vista Senior living communities

Shift: 7:00 PM - 7:00 AM (nights)

Hours: TBD

Vista Senior Living is seeking a skilled Registered Nurse to serve as our overnight Remote Triage Nurse across six assisted living communities in Minnesota. In this role, you are the clinical point person during night hours - assessing changes in resident condition, guiding on-site care staff, making sound escalation decisions, and ensuring residents receive safe, timely, high-quality care when our day clinical teams are off-site. This is a great fit for an RN with strong assessment skills who thrives working independently and making confident decisions.

Job Description

  • Serve as the primary clinical resource overnight for residents across all six communities
  • Receive and triage calls and concerns related to changes in resident condition or status
  • Perform nursing assessments and determine the appropriate level of intervention
  • Provide clinical direction and support to resident assistants, med techs/TMAs, and other care staff
  • Make escalation decisions - continue monitoring, contact the on-call provider, or arrange emergency transfer
  • Communicate with on-call physicians/NPs, families, and incoming day-shift staff
  • Oversee medications and treatments per physician orders as needed
  • Lead the clinical response during overnight emergencies
  • Document all assessments, interventions, and communications accurately and timely
  • Ensure care is delivered in line with Minnesota assisted living regulations (Chapter 144G) and Eden Senior Care policies
  • Provide thorough shift handoff reports each morning

Qualifications:

  • Current, unencumbered Minnesota RN license
  • Minimum 2 year of Triage Nurse experience in Assisted Living, Long‑Term Care, or Skilled Nursing.
  • Strong clinical assessment and triage judgment
  • Ability to work independently with minimal direct supervision
  • Excellent communication skills and comfort directing unlicensed care staff
  • Current CPR/BLS certification
  • Must be proficient using Point Click Care (PCC)
