• Conduct comprehensive psychosocial assessments to evaluate a diverse range of clients' mental health concerns, identifying strengths and challenges to inform care strategies.
• Develop individualized treatment plans that include both client-centered goals and measurable objectives, ensuring they reflect the unique cultural backgrounds and needs of clients.
• Deliver evidence-based therapeutic interventions, such as cognitive-behavioral therapy, motivational interviewing, and family systems therapy, in both individual and group settings.
• Work closely with a multidisciplinary team, including psychiatrists and nurses, to coordinate holistic care and ensure seamless service delivery.
• Provide crisis intervention and formulate safety plans that effectively address immediate risks and enhance client safety.
• Advocate for clients' rights and navigate community resources, helping clients access vital services and support networks.
• Participate in community outreach initiatives to raise mental health awareness and foster an environment of understanding and care.

• Master's degree in Social Work from an accredited institution.
• Active state licensure as a Licensed Clinical Social Worker (LCSW-C).

• Previous experience in a community mental health setting, hospice care, or a related field is highly desirable.
