The Congruent Health Center is actively seeking a dedicated and compassionate Licensed Professional Counselor (LPC) to join our innovative team. Key Responsibilities
  • Conduct thorough assessments to evaluate and address the unique needs of each patient.
  • Maintain clear and consistent communication with supervisors, direct care teams, family members, and relevant stakeholders regarding individual progress and treatment plans.
  • Provide to supervisors a detailed schedule of counseling appointments, home visits, and program observations to monitor patient progress and service efficacy.
  • Coordinate and conduct home visits to assess the environment and review patient progress through observations and discussions with stakeholders.
  • Ensure adherence to therapeutic strategies outlined in patientsโ€™ program plans while evaluating their effectiveness continually.
  • Participate actively in interdisciplinary team meetings, contributing insights and plans that align with our holistic care philosophy.
  • Document and maintain accurate monthly and quarterly progress notes to track patient development and report to relevant parties.
  • Design and conduct training sessions to ensure staff are well-versed in implementing program plans and maintaining accurate documentation.
  • Engage in follow-up interviews and home visits to provide ongoing support tailored to the evolving needs of individuals.
  • Serve as a vital link between the Congruent Health Center and the larger mental health community to promote service development and enhance treatment options.
  • Collaborate in the formulation of training materials to elevate program standards and resources for our clients.
Patient Population / Client Focus

The Licensed Professional Counselor will work with a wide variety of clients, ranging from adults dealing with personal and professional challenges to youth facing transitional hurdles. The patient population reflects an incredibly diverse set of backgrounds and mental health concerns, including anxiety, depression, substance misuse, trauma, and family dynamics, thus requiring a counselor who can adapt their approach to cater to individual and cultural needs.

Qualifications
  • A Master's or Doctoral degree in Counseling, Psychology, or a related field is required.
  • An active and unrestricted licensure as a Professional Counselor (LPC) in California is mandatory.
