Reliable Home Care Providers, Inc. is seeking a licensed Occupational Therapist to provide skilled occupational therapy services to patients in their homes throughout Lake County, Illinois. This part-time, field-based position is well suited for a clinician who values scheduling flexibility, professional autonomy, and the opportunity to build a caseload gradually with the support of an experienced and responsive administrative team.
The Occupational Therapist will work with a diverse adult and geriatric patient population, delivering patient-centered care focused on improving functional independence, safety, and overall quality of life.
Primary Responsibilities

  • Provide direct, skilled occupational therapy services in patients' homes
  • Conduct initial evaluations, reassessments, and ongoing therapy visits
  • Develop and implement individualized plans of care with measurable, patient-centered goals
  • Deliver therapeutic interventions aimed at improving functional independence and safety
  • Collaborate with physicians, case managers, interdisciplinary clinicians, and family caregivers to ensure continuity of care
  • Complete timely, accurate, and compliant clinical documentation in accordance with state and federal requirements
  • Utilize the electronic medical record system (WellSky Home Health) for documentation and care coordination
Requirements
Required Qualifications
  • Current Occupational Therapist license in the State of Illinois
  • Strong clinical judgment, communication, and organizational skills
  • Ability to work independently while maintaining accountability and professional standards
  • Valid driver's license, reliable transportation, and active auto insurance
Preferred Qualifications
  • At least 1 year of home health or community-based therapy experience (preferred, not required)
  • Familiarity with home health documentation standards
