13-Week Contract Home Health Registered Nurse (RN)

Serving Dorchester, Charleston & Berkeley Counties, SC

$45.00/hour | Weekly Pay | Mileage Reimbursement

Interim HealthCare is seeking an experienced Home Health Registered Nurse (RN) for a 13-week contract assignment serving patients throughout Dorchester, Charleston, and Berkeley Counties. This is an excellent opportunity for a confident Home Health RN looking for a short-term assignment with a supportive team, competitive pay, and the opportunity to make a meaningful impact in patients' homes.

What You'll Do:

As a 13-Week Contract Home Health RN, you will:

  • Complete comprehensive patient assessments and ongoing evaluations.
  • Develop, implement, and update individualized plans of care in collaboration with patients, caregivers, and the interdisciplinary team.
  • Provide skilled nursing care as ordered by the physician.
  • Educate patients and families on disease management, medications, and health maintenance.
  • Perform preventative and rehabilitative nursing interventions.
  • Coordinate care with physicians and other healthcare professionals.
  • Monitor patient progress and communicate changes in condition promptly.
  • Supervise home health aides and other paraprofessional staff as appropriate.
  • Participate in Quality Assessment and Performance Improvement (QAPI) activities.
  • Complete accurate and timely clinical documentation.
  • Maintain HIPAA compliance and protect patient confidentiality.

Qualifications:
  • Current South Carolina RN license or Compact RN License.
  • Graduate of an accredited school of nursing.
  • Minimum one (1) year of recent Home Health experience required.
  • Minimum three (3) years of RN experience preferred.
  • Ability to step into an active caseload with minimal orientation.
  • Strong assessment, clinical decision-making, and communication skills.
  • Proficient with electronic documentation and computer applications.
  • Current CPR certification.
  • Valid driver's license, reliable transportation, and proof of auto insurance.
