Position Overview

Seeking an experienced and compassionate Registered Nurse (RN) to provide skilled home health services throughout Thomas, Brooks, and Colquitt Counties. This position is responsible for delivering high-quality patient care, completing comprehensive assessments and OASIS documentation, coordinating care with the interdisciplinary team, and ensuring compliance with all regulatory and agency standards. The ideal candidate will have prior home health experience, proficiency with Homecare Homebase (HCHB), and a commitment to delivering exceptional patient-centered care.

Schedule & Work Details

  • Coverage Area: Thomas, Brooks, and Colquitt Counties, Georgia
  • Schedule: Monday through Friday, 8:00 AM to 4:30 PM
  • Weekly Hour Guarantee: 40 hours
  • Pay Rate: $51.00 - $53.00 per hour
  • Productivity Expectation: 30 points per week
  • On-Call/Weekend Requirement: One weekend every 4 weeks, with the following Tuesday off
  • Mileage Reimbursement: Reimbursed between patient visits

Key Responsibilities

  • Clinical Competence
  • Initiate, develop, implement, and revise individualized plans of care in collaboration with physicians and other healthcare professionals.
  • Provide skilled nursing services in accordance with physician orders, agency policies, and state and federal regulations.
  • Supervise home health aides and licensed practical/vocational nurses, providing instruction, guidance, and task delegation as appropriate.
  • Conduct required supervisory visits and ensure quality patient care delivery.
  • Documentation & Care Delivery
  • Deliver high-quality clinical services within the RN scope of practice while adhering to infection prevention and control standards.
  • Complete comprehensive patient assessments, including OASIS documentation and medication reconciliation, accurately and timely.
  • Maintain compliant clinical documentation that meets agency, payer, and regulatory requirements.
  • Ensure visit documentation is completed and synchronized according to agency policy.
  • Quality & Compliance
  • Perform initial and comprehensive nursing evaluations to determine home health eligibility and medical necessity.
  • Establish and update plans of care in collaboration with physicians and interdisciplinary team members.
  • Ensure documentation supports clinical necessity, regulatory compliance, and reimbursement requirements.
  • Communicate patient needs, changes in condition, orders, OASIS information, and scheduling updates promptly and effectively.
  • Promote patient safety and participate in efforts to reduce avoidable hospitalizations.
  • Teamwork & Collaboration
  • Work collaboratively with the Clinical Director, Executive Director, physicians, therapists, aides, and other clinicians.
  • Participate in agency meetings, education programs, quality improvement initiatives, and survey readiness activities.
  • Assist with orientation and training of new employees and serve as a preceptor when needed.
  • Participate in the agency's on-call and weekend rotation schedule.
  • Support the agency's utilization management processes and performance improvement goals.
  • Patient-Centered Care
  • Provide compassionate care to patients of all ages and backgrounds.
  • Educate patients and caregivers regarding diagnoses, medications, treatments, and self-care techniques.
  • Ensure patients and families receive exceptional service and support throughout their plan of care.
  • Demonstrate respect and professionalism regardless of age, race, creed, color, sex, disability, or financial condition.

Required Qualifications

  • Active Registered Nurse (RN) license, either Georgia RN license or Multistate Compact RN License.
  • Previous home health experience required.
  • Experience using Homecare Homebase (HCHB) required.
  • Strong knowledge of OASIS assessments and documentation.
  • Valid driver's license, reliable transportation, and current automobile insurance.
  • Excellent clinical assessment, critical thinking, and care coordination skills.
  • Ability to work independently and manage a field-based caseload effectively.

Preferred Qualifications

  • Advanced proficiency with Homecare Homebase (HCHB).
  • Strong understanding of Medicare home health regulations and compliance standards.
  • Experience serving as a clinical preceptor or mentor.
  • Excellent communication, time management, and organizational skills.
  • Demonstrated ability to build strong relationships with patients, families, physicians, and interdisciplinary team members.

Dress Code

  • Scrubs or professional clinical attire permitted.
  • No specific scrub color requirement.
