Job description

Assists in performing a variety of registration, insurance verification, scheduling and billing duties in support of inpatient and outpatient access to medical services. The APASR is responsible for engaging eligible patients/members and facilitating enrollment into Health Management programs through proactive outreach, education, and relationship building. This role focuses on identifying eligible individuals, explaining program benefits, and supporting the enrollment process to ensure patients receive appropriate care coordination services. The Outreach Associate prioritizes high-quality patient engagement, accurate eligibility screening, and timely documentation to support operational goals and program growth.

Job Responsibility

  • Assists in performing pre-registration and registration duties; Obtains necessary demographic and insurance information and enters data into computer system; Obtains appropriate signatures on medical, legal and financial forms;  
  • Aids in verification of patient insurance coverage and other related data with third party carriers for scheduled appointments/procedures; Aids in obtaining patient’s insurance authorization required for services; Documents authorization approvals and denials in computer systems; Collects money due from patient at time of service; may preform billing, receivable and related functions. Performs related duties as required. All responsibilities noted here are considered essential functions of the job under the Americans with Disabilities Act.   • Conduct outbound outreach in person or via phone and other approved communication methods to engage patients eligible for Health Management programs. 
  • Clearly explain program services, benefits, and expectations to patients and caregivers.
  • Build rapport and trust with patients to encourage participation and sustained engagement
  • Schedule enrollment appointments or facilitate direct enrollment when appropriate.
  • Address patient questions and provide support throughout the engagement process.
  • Meet established productivity and performance goals related to outreach volume, engagement rates, and enrollments. 

Job Qualification

  • High School Diploma or equivalent required, Associate degree preferred.
  • 0-1 years of relevant experience, required.
  • Experience in healthcare outreach, patient engagement, customer service, or care coordination preferred. 
  • Strong communication and interpersonal skills with the ability to engage diverse patient populations.
  • Proficiency in electronic health records or CRM platforms preferred.
