Occupational Therapist (OT)

Full Time, Part Time and PRN

We are seeking a compassionate and licensed Certified Occupational Therapist (OT) to join our Skilled Nursing Facility rehabilitation team. The Occupational Therapist evaluates residents, develops individualized treatment plans, and provides occupational therapy services to promote independence, safety, and improved quality of life.

Key Responsibilities
  • Evaluate residents and develop individualized occupational therapy treatment plans.

  • Provide therapy focused on ADLs, functional mobility, strength, coordination, cognition, and safety.

  • Monitor resident progress and adjust treatment plans as needed.

  • Complete accurate and timely documentation in compliance with Medicare and facility guidelines.

  • Participate in interdisciplinary care planning and discharge planning.

  • Supervise COTAs as applicable.

  • Educate residents and families on adaptive techniques and equipment.

  • Maintain compliance with federal, state, and facility regulations.

Qualifications
  • Master's or Doctorate degree in Occupational Therapy from an accredited program.

  • Current OT license in good standing in the state of Florida.

  • Skilled nursing or long-term care experience preferred.

  • Knowledge of geriatric rehabilitation and Medicare regulations.

  • Strong communication and documentation skills.
