  • Collaborate with the interdisciplinary team to support safe discharge planning and community re-integration.
  • Document evaluations, progress notes, and discharge summaries per facility and regulatory requirements.
  • Supervise and direct COTAs and rehab aides as part of the OT programme.

Requirements

  • Active, unencumbered Occupational Therapist licence in the applicable state.
  • NBCOT certification required or eligibility.
  • Minimum 1 year SNF or post-acute rehab experience preferred.
  • Current BLS/CPR certification.
