Job description

Job Title: Registered Nurse- Clinical Care Guide
Location: Tucson, AZ 85718 (100% Onsite)
Job Type: Direct Hire | Full-Time
Salary: $85,000-$90,000 per year (W2)
Job Summary:
We are seeking a compassionate and experienced Registered Nurse (RN) clinical to join our care team as a Clinical Care Guide. In this role, you will coordinate specialist referrals, diagnostic testing, chronic disease management, and patient education while ensuring an exceptional patient experience. You'll work closely with providers and patients to deliver high-quality, coordinated care.
Required Qualifications:
  • Active Arizona RN license
  • Associate's Degree in Nursing (BSN preferred)
  • Inpatient Case Management experience (Outpatient Case Management will also be considered)
  • Experience with chronic disease management and patient care coordination
  • Experience coordinating specialist referrals and diagnostic testing
  • Electronic Medical Records (EMR) experience
  • Excellent communication, customer service, and organizational skills
  • Active Arizona Fingerprint Clearance Card (or ability to obtain)
Preferred Qualifications:
  • BSN
  • Case Management Certification (CCM)
  • Experience with healthcare software platforms
