Position Overview

Work Environment

The Physical Therapist role is primarily onsite in patients' homes throughout Browns Mills, New Jersey. Working in the community means you will encounter a diverse range of patient situations, from those recovering from surgery to individuals with chronic conditions requiring ongoing therapy. Key Responsibilities

• Develop and implement individualized treatment plans based on thorough evaluations, focusing on preventing disabilities, relieving pain, and restoring optimal muscle function.
• Supervise Physical Therapy Assistants to ensure the effective execution of treatment plans and compliance with state regulations.
• Utilize diagnostic and functional assessments to evaluate client capabilities, adjusting treatment methodologies as necessary.
• Asses the home environment of patients to identify potential barriers to recovery and recommend modifications to increase client independence.
• Administer various therapeutic modalities, including manual therapy, traction, and exercise programs tailored to each client's needs.
• Collaborate with physicians and case managers to review and update care plans based on client progress and changes, ensuring comprehensive treatment plans that are continuously refined.
• Document patient assessments, treatment plans, and progress accurately and timely in our electronic medical record system, specifically utilizing WebPT for optimal efficiency.
• Train and instruct clients and their families on the proper use of physical therapy equipment and home exercise programs, which ultimately empowers them in their recovery journey.
• Engage in interdisciplinary case conferences to strategize and evaluate holistic treatment approaches for each patient.
• Participate in community outreach programs to educate the public about the benefits of physical therapy and available services.

Patient Population / Client Focus

Our home care Physical Therapists work with a diverse patient population, including elderly individuals, post-operative patients, and those with chronic illnesses or disabilities. Each patient presents unique challenges and opportunities, and it is crucial for the therapist to create a comforting atmosphere while delivering high-quality care tailored to each individual’s circumstances and recovery goals.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited program.
• An active and clean Physical Therapist license in the state of New Jersey.
• Documentation of continued education to stay current with best practices and emerging techniques in the field.
• Ability to effectively use EMR software (e.g., WebPT) for patient documentation and care management.

Preferred Experience

• Previous experience in home care or outpatient settings.
• Strong background in manual therapy techniques and exercise program design tailored for various patient needs.
• Demonstrated ability to work collaboratively within a multidisciplinary team and communicate effectively with patients and their families.
• A proven track record of developing individualized treatment plans that yield measurable improvements in patient outcomes.

Compensation and Benefits

The position offers a competitive salary ranging from USD 100,000 to 120,000 annually, complemented by a full suite of benefits.
